If you are a client of AWS’ Community Veterinary Clinic and would like to request your pet(s)’ medical records, please get started by filling out the form below.
If you adopted a pet from AWS, we emailed medical records to you at the time of your adoption. Should you like another copy, please use this form to request records. Be sure to put your pet’s name when they were in our care (name at adoption). You can also call the Adoption Center to request a copy of the records.
We understand that veterinary care can be expensive, so we’ve compiled a list of financial aid resources. The list includes both local and national resources that may be able to help lessen the cost of you your pet’s care.
