If you are a client of AWS Community Veterinary Clinic and would like to request your pet(s)’ medical records, please get started by filling out the form below.
If you adopted a pet from AWS, all medical records are emailed to you at the time of your adoption. If you need another copy, you may use this form to request records, provided you put the pet’s name when they were in our care. You can also call the Adoption Center to request a copy of the records.
We understand that veterinary care can be expensive, so we’ve compiled a list of financial aid resources, both local and national, that may be of help to you in lessening the cost of you your pet’s care.
